Dr. Kristian Sanchack, MD MHA FAAFP was born in Pennsylvania and graduated from Union College in 1997 with a BS in Biochemistry. He attended medical school at Uniformed Services University of Health Sciences and completed Family Medicine Residency training at Naval Hospital Camp Pendleton from 2002-2005, where he was selected as Chief Resident. Dr. Sanchack was on active duty for 25 years in the US Navy. During that time, he served in various roles providing broad spectrum Family Medicine. His duty stations included Naval Hospital Camp Pendleton, Ca, Branch Health Clinic Iwakuni Japan, Madigan Army Medical Center, WA, Naval Hospital Jacksonville FL, and Commander Naval Surface Squadron-14, Mayport FL. He did 2 tours in Iraq at Al Taqaddum, and Erbil. His favorite military job was the program director of the Jacksonville Family Medicine Residency Program at Naval Hospital Jacksonville, FL. While in that role in 2016, he was recognized by the Jacksonville Business Journal as a local “Health Care Hero” for the larger community. His personal awards include Legion of Merit, Meritorious Service Medal, Navy and Marine Corps Commendation Medal, Army Commendation Medal, and two Army Commendation Medals with Combat “C” Device and Navy and Marine Corps Achievement Medal (two awards); and numerous unit and other awards. After retiring from military service, he started Green Leaf Direct Care, PLLC so that he could practice medicine that allows him to develop a personal connection to his patients at affordable and transparent costs. Dr. Sanchack has published multiple peer reviewed manuscripts, as well as spoken on a range of topics including education, leadership, autism spectrum disorder and complementary care in local to national venues. He is married to Danielle Sanchack, Psy.D, who is a clinical psychologist at Green Leaf Direct Care, PLLC as well! He has two lovely daughters who keep him on his toes.
